New prostate cancer guidelines

The German guideline for the early detection, diagnosis and treatment of prostate cancer is the most comprehensive in the world and of the highest quality (S3). It was first presented to the public on the German Association of Urology congress in 2009.

On 16 September it was finally time: In a press conference, the long-awaited prostate cancer guidelines at the congress in Dresden was presented. Funded by the German Cancer Aid it was created under the auspices of the German Society for Urology and with the participation of numerous other organizations, especially from other disciplines. For more than 70 experts were several years honorary busy with the preparation. You sighted 10,000 works from the literature and evaluated 900 of them patterned from.

The result is a plant with more than 600 pages, the most comprehensive guideline on prostate cancer in the world. The evaluation of the literature resulting probative value (evidence) of the statements in connection with a target at each point consensus of experts gives you the highest level of development (S3). It contains 153 recommendations and 42 comments on the early detection, diagnosis and therapy of prostate cancer, as well as on the topics of rehabilitation, aftercare and prevention.

Needed was a rebuild of the guideline because the fundamentals compared to its 10 year old predecessor have completely changed. Thus, the tumors are now detected at a much earlier stage, and there were great advances in diagnosis and therapy. The most important innovations were presented in a plenary session:

Especially now, a PSA -based early detection as early as the 40th Age recommended to watch the PSA course longer and to be able to specifically interfere with a dangerous tumor in the case of a rapid PSA rise. The aim is to recognize only the need of treatment tumors and thus to reduce the "overdiagnosis" and "overtreatment", so unnecessary biopsies and treatments to avoid (see also discussion about the PSA screening despite positive results ).

Another new point is the inclusion of active surveillance, thus postponing a curative (healing) treatment under close-meshed controls (see also Active surveillance at local prostate cancer ).For this purpose, requirements and inspection criteria have been defined. Overall, the guideline expresses a new understanding of cooperation between physician and patient, namely as a partner in the process of a diagnosis and treatment. So are the education of the patient and to take his attitudes and desires at the center.

In the sole publication of the guideline but it will not stay. You shall be brought in next time for training sessions all persons entrusted with the diagnosis and treatment of prostate cancer doctors near. A short version and a version as a guide for patients are in progress and is expected to appear this year. Finally, it is planned to incorporate new knowledge continuously, so that a "living" guideline is produced.
